The best EHR for therapists in 2026 — an honest guide
I'm a licensed psychologist and I built one of the products below, so this guide has a bias you should know about. It also has something most "best EHR" listicles don't: published prices with dates, the add-on math written out, and honest sentences about when my product is the wrong choice. Verify everything; plans change.
The three questions that decide it
- Do you bill insurance? If yes, the insurance workflow — eligibility, claims, ERA posting, denials — matters more than any other feature, and per-claim pricing beats module subscriptions at low volume.
- Do you use AI notes? On most platforms this is a $15–$59/month add-on per clinician; on some it's included. It moves totals more than base price does.
- Solo therapy practice, or multi-disciplinary clinic? Generalist platforms serve mixed clinics well; therapy-only platforms go deeper on groups, measures, and behavioral-health billing.
When each platform is the right answer
Carepatron — the well-funded generalist
Real monthly price $39 (Plus) to $49 (Advanced — the tier with roles and permissions) per user; the advertised $15.50–$19.50 is a six-month promo on the annual rate. SimplePractice — the most widely adopted
The category leader for a decade: biggest community, template marketplace, ePrescribe add-on, the Monarch directory. Plans $49–$99; AI notes via the Care Aide add-on ($59/month). If you want the option most colleagues use and can price the add-ons, it's a defensible default. Full comparison.
TherapyNotes — admin-heavy insurance groups
Behavioral-health only, $69 solo / $79 + $50 per clinician, free unlimited non-clinical staff seats, 24/7 phone support, and the cheapest claims on this page at $0.14. AI notes are a $40/month add-on. For a large group with schedulers and billers, run their math seriously. Full comparison.
Jane — multi-disciplinary clinics
Superb scheduling and online booking, rooms and packages, beloved support. US insurance billing and AI Scribe are add-ons. If your clinic mixes therapy with physio, chiro, or massage, Jane fits that shape best. Full comparison.

My product, so discount accordingly: $19/month for a solo licensed clinician — group practices $39 + $19 per additional licensed clinician, with pre-licensed associates free — with AI notes (dictate, record, or type), telehealth including groups, group therapy with per-member notes and claims, 15 scored measures, e-signed intakes, and a client portal — all included. Insurance is usage-priced ($0.25/claim, $0.15/eligibility check); card processing is a flat 3.15% + 30¢. Self-serve importers from SimplePractice and Sessions Health. Where it's the wrong choice: you need ePrescribe (we don't offer it), 24/7 phone support (we're founder-answered email), or you're outside the US insurance model.
Do the arithmetic, not the vibes
