Practice Harbor gives clinical supervisors a review queue inside the EHR, and a practice with a licensed supervisor and pre-licensed associates pays $39 a month because associates are free. You review each supervisee’s notes and treatment plans in the chart, then co-sign them or send them back with a comment.

Your review queue

The Supervision dashboard lists every submission waiting on you, with the client, the note type, who submitted it, and when. The sidebar link reads Pending Review with a count, so a backlog is visible without going to look for it.

Feedback that stays out of the client record

Every note under review has a Comments thread between you and the supervisee. Use it to ask a question or teach a point without returning the note. Comments stay outside the clinical record and the client’s chart, and a Request Edits comment posts into the same thread, so the discussion and the decision sit together.

For a general check-in that is not about a specific note, internal team messages work too. They never appear in any client’s portal.

Setting up supervisees

The practice owner or a practice manager gives you the Supervisor role, which can sit alongside a clinician role or be held alone if you carry no caseload in the practice. Supervisees are then linked to you, either by the owner when inviting or editing them, or by the supervisee from their own profile.

You can supervise any number of clinicians; each supervisee has one supervisor. The Supervision column in Settings → Team shows every relationship at a glance, and removing the Supervisor role from someone with active supervisees asks for confirmation first.

Supervision status does not widen anyone’s access. What a supervisee can see is set by their clinician access level, and a common setup for an associate is basic access plus supervision.

Claims and superbills that show the supervision

When a supervised clinician’s session is billed, the claim flags it and lists you as supervising provider in CMS-1500 Box 17, with your NPI in Box 17b. The supervisee stays the rendering provider on the service lines. Your NPI and credentials need to be complete on your profile, because they print on the claim.

A practice setting can also print the supervising provider’s name, license, and NPI on superbills. Payer rules on supervised billing vary, so confirm each payer’s requirements in writing before filing this way.

Hours your supervisees can hand you

Each supervisee keeps a Clinical Hours log in the app. Direct contact counts from completed appointments, and supervision and indirect time are logged by hand. When a board form needs your signature, the supervisee exports the log and brings it to you.

Common questions

How does note co-signing work for clinical supervisors?

Supervisees submit notes for review instead of signing them outright. Each submission lands in your Supervision dashboard queue. You open it, then either co-sign, which finalizes the note with both signatures and your credential, or request edits with a comment, which returns it to the supervisee. Treatment plans submitted for review arrive in the same queue, and a comment thread on each note keeps your feedback out of the client’s record.

How much does Practice Harbor cost for a supervisor with associates?

A licensed supervisor who owns a practice with pre-licensed associates pays $39 a month. That base includes the owner’s own licensed seat, and pre-licensed clinicians add nothing, so a supervisor with six associates still pays $39. Each additional licensed clinician is $19 a month, and practice manager, biller, and scheduler roles do not add to the bill.

Can I supervise without carrying a caseload in the practice?

Yes. The Supervisor role can be combined with a clinician role or held on its own by a supervisor who sees no clients in that practice. Either way you get the Supervision dashboard with your review queue, the comment threads on notes under review, and the ability to co-sign or return notes and treatment plans from the supervisees linked to you.

Can I leave feedback on a supervisee’s note without sending it back?

Yes. Any note under review has a Comments button that opens a running thread between you and the supervisee. The thread stays outside the clinical record and the client’s chart, so you can teach and ask questions without anything entering the legal record. If you do return the note with Request Edits, that comment posts into the same thread.

Does Practice Harbor put the supervisor on insurance claims?

Yes, for sessions delivered by a supervised clinician. The supervisee remains the rendering provider in Box 24J, and you are listed as supervising provider in Box 17 with your NPI in Box 17b, on the printed CMS-1500 and the electronic claim alike. Whether a payer accepts supervised billing is a contract question, so confirm with each payer in writing first.
