We made the pre-licensed tier free because training years are the leanest ones. You are doing licensed-level documentation on an associate-level income, and your EHR should not be another bill. When you license, the price is $19 a month for a solo practice.

Free means the whole product
The Pre-Licensed tier is not a cut-down version. Every plan includes the same features, and there is no client cap. The tier is set from the license on your profile: associate-level licenses such as LMSW, ASW, LAPC, LPC-A, MFT associates, and psychology interns and postdocs all qualify.
A Business Associate Agreement takes effect when you create your account, so you are not keeping client notes in a shared doc or a personal inbox while you wait on paperwork.

Notes your supervisor reviews and co-signs in the chart
When you are marked as under supervision, your sign button reads Submit for Review. Submitting signs the note and puts it in your supervisor’s review queue, and the note shows Awaiting Supervisor Review until they act.
Your supervisor either co-signs, which finalizes the note with both signatures, or requests edits with a comment. Returned notes land in your Notes to revise list with that comment attached, and a Comments thread on the note lets the two of you talk about it without the conversation entering the client’s record. Treatment plans go through the same review.
You can link your own supervisor from Settings → Profile by ticking I’m pre-licensed under supervision and choosing them from the list. The list shows team members in your practice who hold the Supervisor role, so that role has to be assigned first.
Your clinical hours, counted as you go
The Clinical Hours page keeps three buckets. Direct client contact fills itself from completed appointments on your calendar. Indirect work and supervision time are logged by hand in a few clicks. A progress bar tracks your total against the hour goal you set, such as 3,000 hours.
Filters by bucket and date range let you check any slice of the log, and Export produces the log for your board paperwork or your supervisor’s signature.
Insurance billing under a supervisor, filled in correctly
Some payers credential associates directly, some allow supervised billing, and some do not cover pre-licensed clinicians at all. Practice Harbor handles the claim mechanics once you have confirmed what your payer allows.
For a supervised clinician, the claim lists you as the rendering provider and your supervisor as the supervising provider in Box 17, with their NPI in Box 17b, on both the electronic claim and the printed CMS-1500. A practice setting can also print the supervising provider’s name, license, and NPI on superbills. Electronic claims are 25 cents each and eligibility checks are 15 cents.
When you get licensed
Nothing moves and nothing gets rebuilt. Add your full license and NPI in Settings → Profile, and your tier changes to Licensed at $19 a month on the next billing cycle. Your notes, clients, and settings stay where they are.
Records are stored in the FHIR R4 standard and can be exported at any time, client by client, as a zip of PDFs and original documents.
Where Practice Harbor is not the right fit
- If your supervisor will not work inside Practice Harbor, the in-app co-sign workflow is not available to you. You can still document, sign your own notes, and export them, but review happens outside the app.
- Practice Harbor has no place to write notes about the supervision meeting itself or to fill out board supervision forms. Keep those where your board or agency expects them.
- If your practicum site or employer already provides an EHR, use theirs for their clients.
Common questions
Is there a truly free EHR for pre-licensed therapists?
Yes. Practice Harbor is free for pre-licensed clinicians, associates, interns, and trainees. It is not a trial and it has no client cap. You get the same features as paying practices, including AI notes, telehealth, a client portal, billing, supervisor co-sign, and clinical hours tracking. No credit card is required. The tier is set from the pre-licensed license on your profile, and it changes to $19 a month only when you add a full license and NPI.
Does my supervisor have to use Practice Harbor?
Only for the in-app co-sign. To review and co-sign your notes inside Practice Harbor, your supervisor needs to be a member of the same practice with the Supervisor role, and that role can be held by someone who carries no caseload there. If your supervision happens outside Practice Harbor, you leave the supervision box unticked, sign your own notes, and share them with your supervisor the way your site already does.
Can I track my hours toward licensure in my EHR?
Yes. The Clinical Hours page logs direct client contact automatically from completed appointments, and you add indirect and supervision time by hand. A progress bar shows your total against your hour goal, and filters let you review any date range or bucket. When your board or supervisor needs the record, Export produces your hours log for the paperwork.
What happens to my notes when I get licensed?
They stay put. When you add your full license and NPI to your profile, your tier changes from Pre-Licensed to Licensed on the next billing cycle, and the price becomes $19 a month for a solo practice. Your clients, notes, treatment plans, and hours log remain in the same account. Every record can also be exported at any time if you move to another practice.
Can a pre-licensed associate bill insurance in Practice Harbor?
Where the payer allows it, yes. Practice Harbor builds supervised claims with the associate as rendering provider and the supervisor as supervising provider in Box 17, with the supervisor’s NPI in Box 17b. Payer rules vary widely, and some commercial plans do not cover pre-licensed clinicians under any arrangement, so confirm each payer’s rules in writing before you file. Claims are 25 cents each.
I work as an associate in a group practice. Do I need my own account?
Usually not. If the practice uses Practice Harbor, the owner invites you as a team member and marks you as under supervision, and pre-licensed clinicians are free on every plan, so you add nothing to the practice’s bill. A separate free account of your own makes sense when you need a system for clients the practice is not responsible for.
