Clinical picture
- At least one attack followed by a month or more of anticipatory worry or avoidance behavior.
- Rule out cardiac, thyroid, and stimulant causes on first presentation; clients frequently arrive via the ER.
- Interoceptive and situational exposure (CBT for panic) is first-line; document the exposure hierarchy in the plan.
- If avoidance extends broadly to situations where escape feels difficult, assess agoraphobia (F40.00), which is coded separately in ICD-10.
Documentation tips
- Chart attack frequency, triggers (or their absence — unexpected attacks are definitional), and the avoidance pattern.
- Track a panic-specific log or measure; reduced attack frequency and re-approach of avoided situations evidence progress.
