What are Therapy Progress Notes?

Therapy progress notes are clinical documents that record a client's development during treatment. These notes track the client's progress, document interventions used, and outline future treatment plans. Well-written progress notes serve multiple purposes: they provide continuity of care, meet legal and ethical requirements, support insurance claims, and help clinicians reflect on their treatment approaches.
